each agency, and a list of their programs that are at least partly funded by
United Way.

Pledge Form (PDF file)
This form can be used by the employee of an organization conducting an
internal United Way campaign to make a pledge to the current United Way Campaign.
Donor Choice Form (PDF file)
This form can be used to make a donation to a specific organization
associated with the current United Way Campaign.

This form can be used to sign up for the Manley's Mighty Mart incentive
